What is a Primary Care Network?

A primary care network is a group of practices who work together to focus on local patient care. They’ve been around since 2019 and 99 per cent of practices across England now work as part of one.

With increased demand and a shortage of clinical staff, practices find they can provide greater, more personalised care for patients when they can work together on certain services or issues.

Take a look at this short animation from NHS England which explains how they work.

Our History

We are Central Barking & Dagenham Primary Care Network (PCN), a collaboration of 3 forward thinking GP practices serving a population of 30,000 patients from the Barking & Dagenham Area. Our practices include Tulasi Medical Centre, Becontree Medical Centre and Marks Gate Health Centre.

We formed in 2019, when Primary Care Networks were first introduced in the NHS Long Term Plan. The population in Barking and Dagenham is ever increasing with people living longer, and as the complexity of care has increased, the services provided by GP’s have had to transform to account for this.

We’re small enough to deliver the personal care our patients value, yet large enough to make a significant impact through collaboration between our practices and other essential services.

Our Mission

To provide comprehensive, patient-centred care that enhances the health and well-being of our community through collaborative, accessible and high-quality medical services.

Our Vision and Values

Our vision: Create a unified approach in supporting and developing general practice, serving our local communities with trust and collaborative relationships. We aim to deliver population-based healthcare that provides local services for local people, ensuring everyone can start well, live well and age well.

We will achieve this vision by exhibiting our values of:

Patient-Centred Care: We prioritise the needs, preferences, and well-being of our patients in every decision and action

Compassion: We provide care with empathy, understanding and respect for every individual.

Collaboration: We foster teamwork among healthcare professionals, patients, and the community to achieve the best health outcomes.

Innovation: We embrace new technologies and approaches to enhance patient care.

Quality: We are committed to the highest standards of medical excellence and continuous improvement.
